SmileCost publishes independent US dental implant cost ranges so patients can compare quotes before leaving a deposit. We are not a dental practice, insurer, or lending marketplace. The calculator and state pages exist to make fee schedules legible — not to sell treatment.
A complete single-tooth implant is usually three bills: the surgical fixture (often CDT D6010), the abutment, and the crown. Marketing “implant specials” that omit the crown or temporary tooth are common. Our mid-range assumes a standard titanium fixture, stock or custom abutment, and porcelain-fused or zirconia crown in a general or specialty office — without major bone grafting.

Insurance, Medicare, and cash-pay reality
Most employer dental PPOs reimburse major services at about 50% after waiting periods, then stop at a $1,000–$2,500 annual maximum. That means one implant often consumes the entire yearly benefit.
